Unlicensed PracticesThe CALI Unlicensed Practices committee has five members who conduct cursory investigations into complaints of unlicensed activity related to PI or PPO licenses in California.
Complaints are received through the link below or by sending a message to the ULP Committee Chair, Dave Walker, at email@example.com
Initiating a complaint using the link below will direct you to the intake form in TrackOps.
Step 1 – Enter your information as a “new client” on the CLIENT INFORMATION page.
Step 2 – Enter all pertinent information about the complaint in the box labeled NOTES AND INSTRUCTIONS on the CASE DETAILS page.
Complete the intake form and click SAVE CASE REQUEST AND FINISH!
When making a complaint it is important you provide as much information as possible about the potential offending person or agency including name, business name, addresses, telephone numbers or Internet site containing information.
